Transportation


  1. All athletes must take the bus to and from the meets, unless a parent’s signature is supplied via this form prior to the end of the meet. 
  2. Swimmers and Divers must stay for the entire duration of the meet, unless they have spoken to a coach prior to the event about leaving early. Leaving early will only be allowed in extenuating circumstances.

Parent Consent & Liability Release

A Consent and Liability form must be turned into Coach Donna if your swimmer will be driving their own personal vehicle to school or to and from practice.
